Written Request for Release of Information

The Public Records Request Form provides members of the public with a clear and accessible way to request records maintained by the City of Madera, in accordance with applicable public records laws. Pursuant to Government Code Section 6253(c), public agencies have up to 10 days to determine whether a requested record is subject to disclosure under the Public Records Act. If the requested records cannot be provided, you will receive a written explanation stating the reasons for the non-release.

Submitting this form ensures your request is accurately received, documented, and processed in a timely and efficient manner. It allows City staff to understand the specific information you are seeking and to respond appropriately, whether by providing access to the records, explaining any exemptions, or directing you to the appropriate agency or department.

Cost Information

Paper copies are charged at the following rates:

  • $0.10 per page (single-sided)

  • $0.15 per page (double-sided)

Mailing costs vary based on current USPS rates and may depend on the type of materials requested, the volume of records, and other factors.
